Nevada continues to experience persistent and interconnected challenges across the HIV prevention and care continuum, including disparities in diagnosis, linkage to care, retention, and viral suppression that disproportionately affect communities of color, rural residents, and people with overlapping social and structural vulnerabilities. Fragmented planning processes across prevention and care programs, evolving federal requirements, and the growing need to align HIV efforts with broader syndemic priorities underscored the need for a coordinated, statewide approach. In response, federal funders prioritized the development of an integrated HIV prevention and care plan to ensure that resources are strategically aligned, data-driven, and responsive to the lived realities of people affected by HIV in Nevada.
The Solution
Our team led the development of Nevada’s HIV Integrated Prevention and Care Plan by coordinating a comprehensive, inclusive planning process that brought together state and local health departments, community-based organizations, healthcare providers, people with lived experience, and federal partners. We synthesized epidemiologic data, programmatic insights, and qualitative stakeholder input to assess needs, identify gaps, and prioritize strategies across the full HIV care continuum. This process emphasized equity, collaboration, and alignment across funding streams, ensuring that prevention and care efforts were mutually reinforcing rather than siloed. The resulting plan provides a clear roadmap of goals, strategies, and measurable objectives designed to guide implementation, strengthen partnerships, and support sustained progress toward ending the HIV epidemic in Nevada.
The Results
The completed HIV Integrated Prevention and Care Plan was well received by state leadership and federal funders as a high-quality, actionable, and collaborative product that met all federal planning requirements while reflecting Nevada’s unique context. The plan strengthened cross-program alignment and positioned partners to more effectively leverage existing and future funding opportunities, supporting coordinated implementation across prevention and care systems. Importantly, the planning process itself deepened relationships among stakeholders and elevated community voices, laying the groundwork for ongoing collaboration beyond the life of the plan. For funders and partners alike, the project delivered both a strategic planning document and a durable framework for sustained, data-informed action.
